Limb Symmetry Index

Limb symmetry index compares operated and healthy leg performance as a percentage—key ACL metric for strength, hops, and return-to-sport decisions.

Limb symmetry index is the operated leg’s performance divided by the uninvolved leg’s, expressed as a percentage—for hop distance, peak torque on isokinetic testing, or strength tests like single-leg squat max. After ACL rehab, clinicians often require roughly eighty-five to ninety percent symmetry before running, cutting, or full sport return.

Below-target symmetry with good pain scores still signals re-injury risk. Retest after illness or training gaps.

Common questions

  • Does ninety percent symmetry guarantee safe return to sport?

No—it is one gate. Landing mechanics, psychological readiness, sport-specific rehearsal, and clinician clearance complete the decision.
